Voter Registration Application
This is a demonstration of a Voter Registration Application for U.S. Citizens. This type of form can be used to register to vote in a particular state. The online process allows electronic data collection with the security of a signed document. Please note: This is just a demo, you cannot actually register to vote using this application.
Insurance Application - Designation of Beneficiary
This is a demonstration of how Insurance companies can utilize FormFiler for various applications that require signatures while eliminating data entry. This is an example specifically for the designation of beneficiaries, but this same type of application can be used for any forms that require handwritten signatures.

Online Brokerage Account
Application
Many brokerages have begun trading online. To enable online trading, the
brokerage must acquire the customer's signature. In the most common scenario, the customer completes an online form and then waits for a package to arrive in the mail that they must sign and return to their
broker. Using FormFiler, a customer can complete this transaction in one day
since they will instantly receive the form that needs to be signed. They can
print it, sign it and mail or fax (or scan and email) it back to their
broker and begin trading right away.

EZPass is toll collection technology that saves money and reduces traffic congestion. This demo is designed to show how FormFiler can make the
EZPass application easier to complete and faster and less expensive to process.
URLA Mortgage Application
The Uniform Residential Loan Application is used by thousands of people to obtain a mortgage. This online demo works interactively with a customer to obtain complete and accurate information, while expediting the processing of each transaction.
HCFA
1500 Claim Form
The HCFA 1500 is a generic healthcare claim form used to process millions of health-related claims each year. In this example, we use the actual HCFA 1500 to gather information.
Prescription Drug
Form
Our sample prescription drug form demonstrates how pharmacies can obtain an electronic copy of a prescription with the signature required from the doctor. This
results in a computer-readable prescription for the medical office files, eliminates manual prescription entry at the pharmacy, enables pre-insurance verification,
allows pre-filling prescriptions during non-peak hours, significantly reduces the problem of handwriting
interpretation errors, and enables label printing before the patient arrives,
all with the signed, bar-coded prescription form, as legally required.
